Metrovick mishap, May, 1964

Would anyone have any info on the Metrovick seen in these photos beside Sulzer B108? She appears to have serious cab frontal damage and was wondering how this happened? The photograph was taken in Inchicore Works in May 1964. Photos (apologies for quality of copies), from; (Colour-Rail, Huntriss, D., IRISH TRACTION in Colour, Ian Allan Publishing, Surrey, 2011). cant find anything on the net about any accidents in that year . could the incident have happened a few years earlier and she was awaiting her faith?

it could be one of 2 metros, A42 or A15, which collided head on outside of Mullingar in december 1963- seems to be the only reported accident at that time.
