There was a deadman's system on the loco, but it was isolated because the loco brake was applied. The reason the loco kept on going was because it was at full throttle... which, if the dynamic brake had been correctly applied, would've contributed to stopping the train.

Should be able to read PDFs on iPhones and iPads, Des. I post notifications on the Facebook page and on Twitter whenever new stuff is added, which will hopefully prove handy for people.

It happens quite often but normally isn't normally reported. A tough time for the driver, yes, but also for the family of the deceased. It's sad to think that he/she felt there was no other choice. -
